athlete requirements
Under 16 Years Old:

A qualified doctor's** clearance note (Download suggested form here)
16-35:

Physical exam (Download suggested form here) AND a qualified medical doctor's clearance note, and blood test.
36 to 40:
-
ECG (Electrocardiogram). but can be waived by a qualified medical doctor's note to state that based on their professional opinion that such exam is deemed not necessary.

Above 40:

ECG (Electrocardiogram). But can be waived with a qualified medical doctor's written note to state that based on their professional opinion that such exam is deemed not necessary.

Brain MRI. Also can be waived with can be waived with a qualified medical doctor's written note based on their professional opinion that such exam is deemed not necessary.

**A qualified doctor must be a Medical Doctor, or a Doctor of Osteopathic Medicine that holds a current valid medical license to practice in the United States.

preferred lab for blood test: Request-a-test
RequestATest.com is an accepted lab by Arizona Boxing Commission and the preferred lab suggested by the Arizona Sanda Association. They provide a service package called “Combative Trio” which will fulfill the medical exam requirements for the Phoenix Sanda Nationals. The cost for the “Combative Trio” is at a discounted rate of $89.

*Results from blood tests will typically take 1-2 business days, however due to unforeseen circumstances it may take longer. If competitor is currently taking Biotin (also known as vitamin B7 or B8, vitamin H, or coenzyme R) please stop consumption of Biotin at least 72 hours prior to the lab test.
